Colorado Code § 30-10-612

Verdict of jury - form

The jurors, having heard the testimony and having
made all needful inquiries, shall return to the coroner their inquisition in writing, under their
hands in substance as follows, stating the matters in the following form, suggested as far as
found:
STATE OF COLORADO,
County of ..............
An inquisition held at ............, in ............ county, on the ............ day of ............ A.D.
20......, before .............., coroner of said county, concerning the death of .............. or person
unknown, by the jurors whose names are subscribed; the said jurors upon their oaths, do say
............ (here state when, how, by what person, means, weapon, or accident he came to his death,
and whether feloniously).
In Testimony Whereof, the said jurors have hereunto set their hands the day and year
aforesaid.
