Affirmation - form - perjury

(1) A witness who desires it, at his option,
instead of taking an oath may make his solemn affirmation or declaration by assenting when
addressed in the following form:
"You do solemnly affirm that the evidence you shall give in this issue (or matter),
pending between ........ and ........ shall be the truth, the whole truth, and nothing but the truth."
(2) Assent to this affirmation shall be made by answer: "I do."
(3) A false affirmation or declaration is perjury in the first degree.
