(1) To renew a license issued pursuant to this article 205, a licensee must submit an application in the form and manner designated by, and must pay a renewal fee in an amount determined by, the director. (2) Licenses issued pursuant to this article 205 are subject to the renewal, expiration, reinstatement, and delinquency fee provisions specified in section 12-20-202 (1) and (2). A person whose license has expired is subject to the penalties provided in this article 205 or section 12-20-202 (1). (3) The licensee shall submit additional information that the director requests, including evidence that the licensee has maintained and holds a current, valid certification from the national certifying agency, to fully and fairly evaluate the applicant's qualifications for license renewal and to protect public health and safety.
