A consumer shall be referred to a provider of habilitation services under this chapter when all of the following apply: (a) The individual is an adult who has been diagnosed as having a developmental disability. (b) The individual is determined to be in need of and has chosen habilitation services through the individual program planning process pursuant to Section 4646.
