If discrepancies are found to exist during the preenrollment period, the department may conduct additional inspections prior to enrollment. Failure of a provider to remediate discrepancies as prescribed by the director may result in denial of the application for enrollment. The department may deactivate all of the providerâs business addresses if the department determines that the discrepancies are material to the providerâs continued enrollment and the providerâs compliance with program requirements at the additional business addresses.
