(a) The department shall prescribe and provide suitable forms of applications, certificates of ownership, registration cards, driversâ licenses, and all other forms requisite or deemed necessary for the purposes of this code and shall prepay all transportation charges thereon. (b) The department may require that any application or document filed with the department be signed and submitted under penalty of perjury.
