At or before the time a relinquishment is signed, the department, county adoption agency, or licensed adoption agency shall advise the birth parent signing the relinquishment, verbally and in writing, that the birth parent may, at any time in the future, request from the department, county adoption agency, or licensed adoption agency all known information about the status of the childâs adoption, except for personal, identifying information about the adoptive family. The birth parent shall be advised that this information includes, but is not limited to, all of the following: (a) Whether the child has been placed for adoption. (b) The approximate date that an adoption was completed. (c) If the adoption was not completed or was vacated, for any reason, whether adoptive placement of the child is again being considered.
