In addition to the certificate requirements of Sections 12714, 12714.5, and 12715, all of the following information shall appear on weighmaster certificates issued when adjustments are made to the load and the weights are determined at other than the site where the vehicle was loaded: (a) The gross weight of the commodity and the vehicle at the time of the initial weighing. (b) The gross weight of the commodity and the vehicle after the adjustment. (c) The name and address of the seller and of either the purchaser or broker of the commodity as provided by the driver of the vehicle.
