(a) The response by a judgment debtor shall contain a verification that a copy of the response was sent to the claimant or, if the claimant is represented by an attorney, to the claimantâs attorney, at the address specified in the application for the claimant or their attorney. (b) If the judgment debtor is not represented by an attorney in objecting to payment of the application, the response shall contain the judgment debtorâs name, the address at which they wish to receive correspondence and notices relating to the application, and a telephone number where they can be reached during regular business hours. If the judgment debtor is represented by an attorney in objecting to the application, the response shall contain the name, business address, and telephone number of the attorney.
