A petition shall set forth the following: (1) The name, address, and if known, the date of birth of the maltreated adult who shall be designated as the respondent; (2) The maltreated adult's current location; (3) The name and address of the maltreated adult's closest adult relative, if known; (4) (A) The facts intended to prove the person to be maltreated. (B) The facts may be set out in an affidavit attached to the petition and incorporated into the petition; and (5) The relief requested by the petitioner. Acts 2005, No. 1811, § 1
