(a) An applicant for a certificate shall: (1) Submit an application to the State Board of Health; and (2) Pay to the board the application fee established by the board by rule. (b) The application shall: (1) Be on a form and accompanied by any supporting documentation that the board requires; and (2) Be signed and verified by the applicant. (c) The application fee required under this section shall be sufficient to pay for the administrative cost of the certification program and any other cost associated with carrying out this subchapter. (d) (1) All application fees shall be special revenues and deposited to the credit of the Public Health Fund. (2) Any unexpended balance of such fees at the end of each state fiscal year shall be carried forward to the next fiscal year to be used for the same intent and purposes as set forth in this subchapter.
