Massage therapy instructor

(a) A person who holds a license as a master massage therapist issued by the Department of Health and who submits satisfactory evidence to the department that he or she has successfully completed and meets the requirements stated in § 17-86-102 shall be entitled to be upgraded to massage therapy instructor. (b) Each application for upgrade to massage therapy instructor is considered a new application for purposes of criminal background checks. (c) Fees are as follows: (1) Application fee ............... $75.00 Nonrefundable (2) Original license fee ............... 80.00 (3) Biennial renewal fee ............... 80.00 (4) Duplicate license fee ............... 10.00 (5) Pocket card fee not to exceed ten dollars ($10.00).
