(a) An individual licensed or certified under this chapter whose license has been lost or destroyed may apply to the Arkansas Orthotics, Prosthetics, and Pedorthics Advisory Board for a replacement. The application shall be accompanied by an affidavit setting out the facts concerning the loss or destruction of the original license. (b) An individual licensed or certified under this chapter whose name is changed by marriage or court order may surrender his or her license and apply to the board for a replacement license.
