Disclosure of child support obligations

(a) (1) At any time an application is made for workers' compensation, an employer shall require the applicant for compensation to state whether or not the applicant has child support obligations, if the obligations are current or past due, and to whom the obligations are payable. (2) The application shall also include the name of the workers' compensation carrier and the name of the employer. (b) The employer shall forward a copy of the application to the Office of Child Support Enforcement of the Revenue Division of the Department of Finance and Administration. Acts 1993, No. 1185, § 1.
