15-160.04. Routine health care services; employee credentials; training requirements; definition A. On request by a student's parent or guardian, each school district or charter school that provides routine health care services to students in the school's health office shall provide the health care credentials, if any, of each individual who provides routine health care services to students in the school's health office. B. On request by a student's parent or guardian, each school district or charter school shall provide information describing the types of emergency response training, including cardiopulmonary resuscitation training, that the school district or charter school requires school employees to complete. C. This section does not require a school district or charter school to release the personally identifiable information of a school employee or an individual who provides routine health care services to students, including the employee's or individual's address, salary, social security number or telephone number. D. For the purposes of this section, " health care credentials" means a license, certificate, permit or registration to practice a health care profession under title 32.
