A person may enroll in a state plan by applying to the plan administrator. The application must include the following: (1) name, address, age, and length of residency of the applicant; (2) a designation of the plan desired, including deductible option chosen; (3) information relevant to whether the person is a high risk or a federally defined eligible individual; and (4) payment of the first premium.
