(a) A tanning facility shall not allow a minor between the ages of 14 and 17 to use a tanning device, unless the facility has on file a statement signed by the parent or legal guardian of the minor stating that the parent or legal guardian has read and understands all warnings the tanning facility is required to post, consents to the minor using the tanning device, and agrees that the minor will use the provided protective eyewear. (b) A minor under the age of 14 shall be accompanied by a parent or legal guardian on the initial visit of the minor and shall give written permission for the minor to use a tanning device.
