The State Board of Health may charge fees in accordance with the following schedule based on the cost of building construction: Construction Cost Fee (percent of construction cost) Up to $100,000 Greater of $500 or 1.2% $100,001 - $1,000,000 Greater of $1,200 or 0.5% $1,000,001 - $5,000,000 Greater of $5,000 or 0.2% $5,000,001 - $15,000,000 Greater of $10,000 or 0.1% Over $15,000,000 $15,000
